Appointments

While we make every effort possible to see you at your scheduled time, it is sometimes impossible to avoid delays due to emergencies and due to unexpected developments in the situation of patients seen before you. We appreciate your understanding is this area. If an appointment needs to be canceled we appreciate as much notice as possible but at least the minimum notice of 24 hours. This courtesy on your part makes it possible for us to give that time slot to another patient in need. A charge may be applied for all appointments not kept. Please note that a missed appointment charge is not a professional service charge and is therefore not covered by your insurance. Repeated late cancellation of appointments and/or failure to keep scheduled appointments by a patient may make it impossible for us to continue serving that patient.

Telephone Calls

Most significant medical questions need face-to-face appointment. Kindly tell the receptionist your daytime as well as evening phone numbers when calling our office because we may answer non-urgent calls later in the day. Sometimes we call and there is no answer and no answering machine. If your call has not been answered on the day you call, we may have called when you were away. In that situation, please do not hesitate to call us the next business day.

Prescription Refills

Medication refills require two working daysí notice. A charge may be applied for medication refills needed because of missed appointments. Schedule 2 controlled substance refills (for example, Ritalin, Focalin, Concerta, Metadate, Adderall, Vyvanse, etc.) cannot be called over the phone.

When Dr. Rustagi is Away

We make coverage arrangement with local psychiatrists. Covering psychiatrist may be reached through our regular office number or after hours answering service.

Payment Policy

Payment is expected at the time of service, unless prior arrangements have been made at our office. For your convenience, we accept Visa, MasterCard, and Discover cards. If you have serious financial hardship, call us right away. Please don't ignore your balance. Suitable payment plans can be made in such circumstances.

We accept Medicare assignment and Indiana Medicaid

It may be helpful for you to call your insurance company in advance to check you mental health benefits which may be different than you regular benefits. In particular please check how much of your yearly deductible amount (what you pay out of pocket before insurance kicks in) remains to be met and what your mental health visit co-pay is.

We expect our patients to pay at the time of service unless prior arrangements have been made. We are happy to file your primary insurance claim for you. If a monthly account statement has to be mailed, there may be a charge for it.

Initial Appointment

Please bring the following to your initial appointment

A complete Medication list or prescription bottles

Copy of any pertinent Medical Records in your possession including any past psychological testing

For minor students, a narrative school behavior report or a symptom checklist completed by main teachers

Name, address, phone number and fax number of all current doctors and psychotherapists as well as all past psychiatric treatment providers for at least past 2 years

Your insurance card
